I know, it frustrated me too. It was a difficult decision, because if we kept the audio, it probably would have seemed cheesy. And, as my peers pointed out, by not specifying what happened, we leave it a little more open ended, not excluding any other types of accidents.
But to answer your question, we didn't actually script how she died.
