Generally, the more a test plays on the human memory deficit and the more it requires technical accuracy, the harder it is. When I was taking my general/credit tests, I answered questions in full, but lost marks for tiny things that don't matter, ie in english, I'd write out a full explanation of the text, but lose marks for mis-spelling a word or using a dash in the wrong place.

...or is it maybe that the a lot of instructors/teachers are not qualified to teach at the level they are. but if you read the books the answers are all there, it just depends on the student.

Wrong. Couple of years ago, the pass-mark of an exam at my school was lowered because they'd issued one that was too hard. And what about people who do study and fail, or those who never study and pass?
